Summer Medical Services Fee

I am requesting access to services from the UT Dallas Student Counseling Center and/or the Student Health Center during the summer term.

I certify that:

• I was a student enrolled in the spring semester at UT Dallas,
• I am not enrolled for the summer,
• I will be enrolling for the fall semester.

I agree to pay a one-time fee equal to the Medical Service Fee paid by all enrolled students (currently $34.30), which will allow me to receive services from both the Student Counseling Center and the Student Health Center during the summer semester only. I understand that payment of the fee is required before I receive services from either the Student Counseling Center or the Student Health Center.

I understand that this privilege is extended as a courtesy to individuals who were enrolled, fee-paying students during the preceding spring semester and who plan to be enrolled in the following fall semester.
